Need an official price quotation?

A medical assistance application needs an official quotation for the medicines, and we issue one on request: send us the prescription, the patient's name and a mobile number, and we send the quotation back, or deliver it to your home. PCSO asks for three quotations from different suppliers; ours counts as one of them.

How to apply for the guarantee letter

Ask your doctor for three documents, each with their full name, signature and PRC licence number: the prescription, the treatment protocol, and a medical abstract or clinical summary.
For DSWD (AICS): add a copy of the patient's government ID, a Certificate of Indigency from your barangay, a Social Case Study Report from the City Social Welfare and Development Office, and one official quotation from us. Apply at the nearest DSWD office.
For PCSO (MAP or IMAP): add the PCSO application form, the patient's ID and three quotations from different suppliers, and apply on the PCSO website.
When the guarantee letter is approved, send it to us and we prepare the medicines.
